  1. Have I attained the goal which I established as my objective for this year? (You should work with a definite seasonally objective to be attained as a part of your major training objective).  
  2. Have I performed the best possible output of which I was capable, or could I have improved in any areas?
  3. Has the spirit of my conduct been harmonious and cooperative at all times?
  4. Have I permitted the habit of procrastination to decrease my efficiency, and if so, to what extent?
  5. Have I been true to my PERSINALITY, and if not, in what ways?
  6. Have I been persistent in following through with my plans?
  7. Has my relationship with other members and my coach been 100% pleasant and productive? If not, how can I improve with this?
  8. Have I dissipated any of my energy through lack of concentration or effort?
  9. Have I been open-minded towards my training structure?
  10. In what way have I improved in my training?
  11. Have I expressed, either openly or secretly, any form of egotism?
  12. Has my conduct towards other members and my coach been such that it has induced them to respect me?
  13. Have my opinions and actions been based upon guesswork, or accuracy of analysis and thought?
  14. How much time have I devoted to unrewarding effort which I might have used to better advantage?
  15. How may I restructure my time, and change my habits so I will be more efficient during the coming season?
  16. Have I been guilty of any conduct which was not approved by my conscience?
  17. In what ways have I greater contribution to the team than I was expected to?
  18. Have I been unfair to anyone, and if so, in what way?
  19. If I was my coach, would I be satisfied with my progress?
  20. Has another team member been impressed with the help I’ve provided them and if not, why not?
  21. What is my present rating on the fundamental principles of success? (Make this rating fairly, and frankly, and have it checked by someone who is courageous enough to do it accurately).
